Abstract

Gluten consumption has been associated with a spectrum of diseases called “gluten-related disorders”. This review highlights the epidemiological, pathophysiological and diagnostic features of celiac disease and non-celiac gluten sensitivity. Celiac disease results from a complex interaction between genetic factors (specific HLA alelles) and environmental factors (gluten intake); diagnosis relies, in most cases, on serological tests and intestinal biopsy. Non-celiac gluten sensitivity is a non-allergic, nonautoimmune disease of increasing importance. Diagnosis is based on exclusion of associated diseases and confirming that the patient’s symptoms are linked with gluten consumption. While both may have similar and overlapping clinical manifestations, a systematic assessment enables effective differential diagnosis.
